Project Description: The local Yelizovo Health Unit seeks to improve diagnostic, preventative treatment and healing of children in the District. Health Officials identified cardiac treatment , prevention of communicable diseases and respiratory diseases as the most in need of support due to the high number of children with problems in these areas. A cardiac monitor is the major component of the project, training will be done by doctors from the Kamchatka Regional Hospital who have experience in using the equipment. |

Brief Description of Original Project Description: To provide cardio-monitoring and diagnostic equipment for the Yelizovo Children’s Hospital Yelizovo, Kamchatka. Also to provide medical equipment for this same department to be used for care of hospitalized economically disadvantaged, abandoned, and orphaned children.
2.Project Effectiveness
Did the project meet its planned objective? XX YES No
If yes, please explain how the project met its objective: Prior to this project any tests for children with heart anomalies or breathing problems were sent to the Kamchatka Regional Hospital 25 km away in Petropavlovsk-Kamchatsky where the tests were fee-based. Now these tests are available for free in Yelizovo for all children in the Yelizovo District. Also for the first time, the Children’s Hospital can create a computerized data base of it’s patients including electroencephalograph results. For patients with hear problems they can be equipped with a 24-hour a day monitor which they can wear for 3 days to give their doctor a constant picture of the daily stress level on their heart. The office of the Children’s Hospital has now received it’s first computer and printer for use in all kinds of administration. Rotary Club of Yelizovo Nadejda Kulkina who is also a pediatrician, was involved throughout the project planning and implementation. Four members of the YRC attended a dedication of the room. In addition due to some economizing in purchase of computer equipment, the project was able to supply 2 additional computers. One is being used in the Children’s Hospital administration office to computerize the patient records for the first time. The second has been installed in the Children’s Polyclinic to do the same. Also a modem was able to be purchased and needed supplies to connect the computers to the Yelizovo Hospital computer network. Additionally 2 extra printers were able to be purchased for the exam room in order for each piece of equipment to be able to print it’s own report independently. A modem was purchased in order to assist with linking the computers. A scanner was also added to enter in the patient records into the computer system.

Describe in detail how the Rotarians within the project country were involved in the project implementation, including information regarding how many times they communicated with the international sponsor, how many times they visited the project site and any other activities. Yelizovo Rotarians brought visiting Canadian Rotarians to see the future project site. Nadejda Kulkina, a Rotarian and Pediatrician in Yelizovo was on the hospital committee which decided what to buy and what kind of repairs were needed in the room to complete the project. Dr. Yuri Igolkin who is President of the Yelizovo RC completed all bank wire transfer arrangements with our local bank and accounted for the fund disbursal to the Hospital Hard Currency Account. Martha Madsen served as project interpreter, translating all correspondence between in-country club of Yelizovo and sponsor clubs from Canada.

Please note that the Trustees of the Rotary Foundation expect the international co-sponsor to participate in each Matching Grant project in ways other than their financial contributions.
Describe in detail how the Rotarians outside the project country were involved in the project’s implementation, including information regarding how many times they communicated with the local sponsor, whether or not they attended any major project events, if they sought out donations above and beyond the grant funds, and/or any other activities. Under the coordination of Bernie Carrierie the President of the Lethbridge Rotary Club in Alberta, Canada, other club sponsors in the district were located. These include: Airdrie Rotary Club, Drumhjeller Rotary Club, Swift Current Rotary Club, and PDG Bill Gillot. The Lethbridge RC held a black tie bingo fund raising event to gather funds for this project. Also this club submitted a grant to the Province of Alberta the Wild Rose Foundation to help match their contributions. Bernie Carrierie and the Yelizovo Rotary Club exchanged over 50 letters over the course of the project to resolve all questions about administration of the grant from RI.
Please describe how any non-Rotarians and other organizations were involved in the project and how the Rotarians interacted with them. Staff members of the Yelizovo Children’s Hospital assisted with the design of this project. The medical staff ordered and purchased the equipment and installed it in the room. The Yelizovo District Hospital also contributed to the project a work table, coat rack, and of course the room for a new cardiac diagnostic facility at the Children’s Hospital.
Financial Statement
A completed, accurate financial statement is vital for the proper closure of the grant files. Failure to accurately report all expenditures incurred will result in the delay of the final report processing and may result in limitation on future grant projects involving all sponsors/co-sponsors until the matter is resolved.

Copies of receipts for all items with a cumulative or individual cost of US $1000 or more corresponding to these expenses must be attached. Sponsors and/or co-sponsors must maintain original receipts for all expenses incurred for a minimum of three years should an audit be required. 5. Project Publicity
The public Relations Department of The Rotary Foundation and Rotary International are always looking for success stories and action photos to publicize the good works of Rotarians around the world. Please attach copies of any local media coverage the project received, a narrative of the project’s implementation and success, as well as any action-oriented photos to this report (note that photos cannot be returned.) If we require additional information for a publicity story, your club district will be contacted.

Hello Bernie,
Nadejda Kulkina coordinator for the Children's Hospital project says that all of the equipment has been received for the Cardiac Project except the Cardio monitor. She will check to find out when it is expected. We are ordering a commemorative plaque with the names of all the Rotary Clubs involved and the Wild Rose Foundation as well. When the cardiomonitor is received and we get the Rotary emblems to affix on each piece of equipment, we will invite the District Mayor and club members to dedicate the new equipment at the hospital. We'll of course take lots of photos. So I will be in touch when I know about the monitor. Best, Martha

Tonight at our Rotary Meeting, the head doctor of the Yelizovo Children's Hospital came to invite all of the club to the official dedication of the Cardiac Project on January 4, 2003 at 3:00 PM. She gave a very good accounting of the project and said the last few details are being put into place. They have already sent one doctor to St. Petersburg to receive training in how to use the equipment, and so they are all ready to go. She explained that many of the tests now available were formerly only available at the Regional Hospital in Petropavlovsk-Kamchatsky and for this parents paid 200 rubles on average per test. Many parents couldn't afford this plus the trip. Now it will be available for free, right here at home. Each year, just over 1000 children are hospitalized in this facility and now the services they will receive will be much better. It has been our pleasure to collaborate with your Rotary Clubs in Canada and the Wild Rose Foundation. We will take many photographs and complete our report in a timely manner so that you all will be able to share in our joy at this very important addition to health care in the Yelizovo District.
